Role Overview  

The ABI Case Manager works as part of the Cota/ West Park Partnership Team providing short term Case Management assisting persons living with an ABI in the community.  The ABI Case Manager utilizes knowledge and skills in order to perform the following activities:

  • Connecting/engaging with clients,
  • Collaborating with the Behaviour Analyst in developing goal plans with the clients
  • Teaching skills and compensatory strategies to assist clients in living with their ABI
  • Supporting individual recovery and assisting clients to link with and/or apply to other clinical services and/or community programs as needed/desired. 

About the Team

The Cota/ West Park Healthcare Centre ABI Team is comprised of a Cota ABI Case Manager, West Park Healthcare Centre Behaviour Analyst, Cota Individual Support Worker and a Psychologist consultant.

The ABI BT Team works in alliance providing in-home consultation for clients, family and caregivers to help manage challenging behaviours following an ABI. 

Primary Responsibilities: 

  • Conduct face-to-face assessments with service users at their locations including but not limited to home, shelters and hospitals.
  • Interact directly with referral source, physicians, and other services involved in the service user’s care.
  • Initiate services to service users at the point of intake to ensure that proper assessment and initial service response occurs.
  • Develop care plans and establish goals with service users.
  • Collaborate with other systems, organizations and individuals from courts, probation, short-term residential beds program, hospital and community to ensure a well-coordinated service system.
  • Record service user status/progress using the appropriate electronic charting system.

The Successful Candidate Requires:

  •  A combination of a post-secondary degree or diploma in Social work, Social Service Work, or related field, and one-year experience in community services supporting service users with  ABI and or Mental Health.
  • Preference will be given to individuals holding a BSW/MSW or Neuroscience degree and or who have specialized education/training/ certification in the field of Acquired Brain Injury. For example: neuro-rehabilitation, cognitive interventions, neuropsychological assessment, principles of applied behavioral analysis, group facilitation, OBIA’s Brain Basics Training Program, or Level 1 or 2 certification, Behavioural Science Technology.

AND

  • Sound clinical knowledge and understanding of the service user population and demonstrated ability to work independently and effectively with individuals with ABI, mental health, and complex needs.
  • Knowledge of the principles of case management theory and practice using a service user- recovery oriented philosophy.
  • Demonstrated ability to use problem-solving techniques to identify and assess service user needs and develop appropriate intervention strategies.
  • Proven skills in working with challenging behaviors and high-risk environments.
  • Demonstrated ability to effectively manage crisis or conflict situations and solid skills in de-escalating crisis situations.
  • Knowledge of community services and resources specific to ABI.
  • Excellent clinical judgment and decision-making skills, including a keen awareness of client-clinician boundary issues.
  • Ability to function well in team environments and to act as a resource to colleagues. 
  • Open to completing required training if needed.
  • Proficient in MS Office.

Other Requirements and Working Conditions:

The ABI Case Manager is community based, working and supporting service user’s residing in the City of Toronto catchment.

  • Conduct visits in the service users’ homes, hospital, hostels, and other community settings as well as virtually where required.
  • Traveling to multiple locations between service user appointments within a course of a day.
  • Be comfortable working with a broad range of service user groups.
  • Must have access to a vehicle (Please note that access to a vehicle means owning, borrowing, or renting a vehicle. Alternatively, it can include using ride services such as Uber, cabs, and taxis at no cost to Cota).
