Role Overview

The Registered Nurse or Registered Practical Nurse, Case Manager provides case management support and care for the over all well-being of clients.

 
About the Team

The AHFT utilizes an interdisciplinary team and Housing First approach to support individuals who are homeless and living with mental health challenges, and many are involved with a mental health court diversion program and/or are on release from a provincial correctional facility.

The Assertive Housing First Team is an interdisciplinary team comprised of Case Managers who have expertise in Mental Health, Mental Health & Justice, Addictions and Nursing.

The RN or RPN Case Manager will contribute to the development of comprehensive client-centered psychiatric and physical health assessments and treatment plans and provide community-based case management supports to an assigned group of clients, including education, crisis intervention, and linking to community resources. This position also will provide social/ educational and addiction group activities. 


Primary Responsibility:

Clinical Skills:

  • Reviewing the referral information of applicants to the program, assess eligibility for the program and confirm acceptance of new clients into the program.
  • Developing a collaborative, supportive and therapeutic relationship.
  • Completing a comprehensive psychiatric and physical health assessment of the nature of each client’s unique goals and needs through collaborative engagement with each client and their respective support networks.
  • Providing guidance and assistance to each client in relation to the development of both a personalized Client Safety Plan and a Recovery/Goal Plan.
  • Provides health education to clients and team to promote health and well being.
  • Provide direct practical support, coaching and counsel to clients in their pursuit of identified goals and needs. 
  • Support administration of medication program in collaboration with AHFT psychiatrist, including administration and delivery of medications and administration of intramuscular injections.
  • Assisting clients with emergency and/or crisis needs as required/appropriate.

Documentation and Data entry:

  • Documents all client contacts/interactions in the appropriate client chart according to Cota’s documentation guidelines and College standards.
  • Assists with other data collection activities within the program, as determined by the Program Manager.

Administrative tasks:

  • Participates in team meetings, provides input and ideas for improving services, efficiencies and ongoing program planning.
  • Supports creation of team schedule and client appointments.
  • Participates in any research-related activities.


The Successful Candidate Requires:

  • A Bachelor’s degree or Diploma in nursing from a recognized University or College and must be registered with College of Nurse of Ontario.
  • The ideal candidate will have 2 years direct experience in community mental health nursing.
  • Sound clinical knowledge and experience in case management with the client population, including individuals experiencing serious mental health issues, addictions, justice system involvement and homelessness.
  • Demonstrated strong commitment to the right and ability of each person with a serious mental illness to live in normal community residences, work in market jobs, and have access to helpful, adequate, competent, and continuous supports and services.

Demonstrated ability to:

  • Conduct home visits and to exercise judgement with a high degree of autonomy.
  • Establish supportive, trusting relationships with persons with a serious mental illness with respect for clients’ rights and preferences.
  • Use problem-solving techniques to identify and assess client need and develop appropriate intervention strategies.
  • Assess mental health needs, physical health needs, functional capabilities and care options.
  • Ability to make appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric and medical treatment.
  • Offer groups and individual services to educate clients, families and other staff about health and wellness behaviours (e.g., diabetes management).
  • Handle ambiguity to function effectively while dealing with competing priorities.
  • Familiarity with the Ontario Mental Health Act and Mental Health Reform.
  • Knowledge of Recovery as a guiding principle for assessing and developing treatment plans for clients.
  • Ability to work with agencies, hospitals and the broader community.
  • Knowledge of trends and issues related to the delivery of services to a mental health client population.
  • Excellent organizational and interpersonal skills.
  • Excellent computer skills (e.g., Word, Excel, Microsoft Outlook).
  • Ability to manage and adapt positively to an ever changing health care/work environment.
  • Effective communication and written skills.
  • Experience working in a community agency an asset.


Other Requirements and Working conditions:

  • Visits will be conducted in the clients’ homes, hostels, hospitals and other community settings.
  • Weekend and evening rotation is required.
  • Must have access to vehicle.
