Role Overview

 

The Personal Support Service Worker provides tenants with assistance in the areas of personal care, routine activities of daily living (e.g., cleaning, cooking, bathing, laundry, etc.) and supports with pest control preparation (as needed), while providing social and emotional support for tenants residing in the modular home. All activities are performed in a supportive manner to encourage the development of each tenant’s personal ability and independence.

 

 

About the Team

 

We require an experienced Personal Support Service Worker to work as a team member in our Modular Housing program at 321 Dovercourt Road. This site is a part of the City’s Phase 1 Modular Housing Initiative. This site provides permanent affordable housing with supports to people who are exiting chronic homelessness.

Each tenant has a bachelor apartment within the building, access to a meal program offered by the Property Management program TNG (The Neighborhood Group), and onsite support provided by Cota's supportive housing team. Our comprehensive approach to supportive housing has the potential to significantly enhance the tenants' quality of life and promote their overall wellness.

 

Primary Responsibilities:

  • Assist with the development of client’s support/goal plans. 
  • Directly support clients on Activities of Daily Living (ADLs) such as: personal hygiene routines, laundry routines, maintaining and/or cleaning client’s rooms, support clients with their medication administration, and assisted daily living as identified by the Supervisor and/or Team.
  • Support the maintenance of a safe/clean environment.
  • Coordinate and support other activities with the team, including planned outings and group activities
  • Documentation and data entry.
  • Participate in team meetings and provide input and ideas for improving services, efficiencies and ongoing program planning.

 

The Successful Candidate Requires: 

  • A minimum of High School Diploma required as well as certification as a PSW or Health Care Aide.
  • Experience working with clients with an intellectual and/or developmental disabilities, as well as behavioral challenges.
  • Ability to provide personal care (i.e. toileting, bathing, safe lifts and transfers) and other activities of daily living and household management (i.e. cooking, cleaning, assisting with laundry, grocery shopping, guiding them around the community).
  • Ability to lift 10-25 lbs.
  • Demonstrated understanding of ABI symptoms and strategies.
  • Good interpersonal and communication skills (verbal and written).
  • Basic computer literacy including but not limited to Microsoft Word, E-mail, Internet.
  • Ability to exercise good judgment
  • Ability to work independently and within the team setting.
  • Effective communication skills to interact with team, case managers, program manager, etc.
  • Excellent client service approach.
  • Sensitive to challenges experienced by marginalized groups such as poverty, mental health, Acquired Brain Injury, and accessibility issues.
  • Valid Crisis Prevention and Intervention Certificate (CPI), First Aid/CPR Certificate, ASIST and Safe Food Handling Certificate preferred. 

Other Requirements and Working Conditions:

  • Availability to work day, evening and weekend shifts.
